Minimum identity requirements

You must provide four documents with your completed form to confirm your identity:

The documentation you provide must include evidence of your full name and date of birth and a photograph of you. The commencement and primary documents provided must include a photo of you. If your commencement and primary documents do not include a photograph you must submit a passport-style photograph certified by a person listed in Schedule 2 of the Statutory Declarations Regulations 1993 (Cth).

You will need access to a scanner or a mobile device with a camera to upload your ID documents.

At least 1 'Commencement of Identity' document
  1. Full Australian Birth Certificate (not an extract/birth card)
  2. Australian Citizenship Certificate
  3. Australian Passport (not expired/cancelled)
  4. Australian Visa (current at time of entry to Australia as a resident or tourist)
  5. Certificate of Evidence of Resident Status (government issued)
  6. Certificate of Identity issued by the Department of Foreign Affairs and Trade (DFAT) to refugees and non-Australian citizens for entry to Australia
  7. Document of Identity issued by DFAT to Australian citizens or persons who have the nationality of a Commonwealth country for travel purposes
  8. ImmiCard issued by the Department of Immigration and Border Protection that enables the cardholder to prove their visa and/or migration status and enroll in services
At least 1 'Primary Use in the Community' document
  1. Current Australian Drivers Licence, learner permit or provisional licence issued by a state or territory, showing a signature and/or photo and the same name as claimed
  2. Australian Marriage Certificate issued by a state or territory (church or celebrant-issued certificates are not accepted)
  3. Foreign/International Passport issued by a country other than Australia with a valid entry stamp or visa
  4. Proof of Age or Photo Identity Card issued by an Australian government agency in the name of the applicant, with a signature and photo (must be current)
  5. Shooters or Firearms Licence showing a signature and photo (not minor or junior permit or licence, must be current)
  6. Student Identification Card (current) with a signature or photo (for persons under 18 years of age with no other Primary Use in Community Documents)
At least 2 'Secondary Use in the Community' documents
  1. Academic Transcript from an Australian university (certified)
  2. ASIC (Aviation Security Identification Card)
  3. Australian Defence Force Photo Identity Card
  4. Australian or State/Territory Photo Identification Card (government issued)
  5. Australian Secondary Student Photo Identity Document
  6. Australian Tertiary Student Photo Identity Document
  7. Bank Card
  8. Bank Statement
  9. Centrelink Health Card or Veteran’s Affairs Card (evidence of right to an Australian government benefit)
  10. Certificate of Identity issued by the Department of Foreign Affairs and Trade (DFAT)
  11. Consular Photo Identity Card issued by the Department of Foreign Affairs and Trade
  12. Credit Card
  13. Credit Reference Check
  14. Document of Identity issued by DFAT
  15. Enrolment with the Australian Electoral Commission
  16. Foreign Government Issued Documents (e.g. Foreign Drivers Licence)
  17. Medicare Card
  18. MSIC (Maritime Security Identification Card)
  19. Police Force Office Photo Identification Card
  20. Private Health Insurance Card (Australian)
  21. Rates Assessment Notice or ATO Assessment Notice
  22. Security Guard or Crowd Control Photo Licence
  23. Trade Association Card (Australian)
  24. Trusted Referees Report
  25. United National Convention Travel Document Secondary
  26. Utility bill (Australian only, must show name and address)

Change of name

If you provide identity documents using a former name, you must provide evidence of your name change. This means providing a change of name certificate issued by the Australian Registry of Births, Deaths and Marriages or an Australian marriage certificate issued by a state or territory, in addition to your four identity documents. Church or celebrant-issued certificates are not accepted.

Special provisions for proof of identity
